  Manual is now available. You can access the Manual by
  logging into your MBI account.

  We hope that you will take some time to look through
  the document in order to familiarize/re-familiarize yourself
  with our provider policies and procedures that are currently in
  effect. Please note that the Manual has separate sections for
  Kentucky, Indiana and Tennessee, and you are encouraged
  to look at the Table of Contents to determine which sections
  apply to your state, and review the information within those
  sections.
